Mustaine Award – This is the highest honor that KY SHAPE bestows. The recipient should have been recognized at some earlier date. It should be an award of great honor and the person must be carefully chosen.
Life Member Award – This is awarded to someone who has had a long period of service (at least ten years) or who may have retired. It recognized distinguished service to the profession.
Distinguished Service Award – This is awarded to members of KY SHAPE in recognition of meritorious service over an extended number of years.
Merit Award – This awarded to members and non-members of KY SHAPE. It is to recognize merit in the fields of HPERD and fields of allied interests. It may be presented to persons, schools or institutions/corporations.

The Kentucky SHAPE Teacher of the Year is sponsored by OPEN and US Games. The program recognizes outstanding teachers in the following categories:
- Adapted Physical Education (K-12)
- Elementary Physical Education (K-5)
- Middle School Physical Education (6-8)
- High School Physical Education (9-12)
- School Health Education (K-12)
- Dance Education (K-12)
Individuals who are named KYSHAPE Teachers of the Year are eligible the following three years after their award for SHAPE America’s District Teacher of the Year award. District winners are eligible for the national award and the same eligibility rules are applied at the national level as the district level.
